To improve and protect the health of Sparta Township School District students, the Health Services Department exists to provide for the well-being of students so they may further their education.  To do so requires the authorization and consent of the parent/guardian, and in certain circumstances a physician, as well.  This page contains the forms that your child may need in order to receive certain services while in school.
