Athletics Information and Forms

  • Attention Sparta High School's Incoming Freshman (Class of 2023) and Returning Athletes:

    All SHS athletes participating in a 2019 Fall sport MUST have their physical forms into the Nurse's Office NO LATER than July 19, 2019.  If you are submitting your physical paperwork after June 21, 2019 and during Summer months, please drop this paperwork in the Main Office of Sparta High School.  This paperwork should not be submitted to the Athletic Department or the Athletic Trainer.   *NOTE: Forms submitted after that date may cause a delay in the athlete's participation in practices.  

    The NJSIAA (New Jersey State Interscholastic Athletic Association) has issued a new "Preparticipation Physical Evaluation" form for the 2019-20 school year.  The packet consists of 4 pages total, pages 1 & 2 (Health History) to be completed by parent/guardians and reviewed by the doctor, and pages 3 & 4 (Physical Examination Form) to be completed and signed off by doctor. The state now requires that all examining physicians have completed the Student-Athlete Cardiac Assessment Professional Development Module. All physicals are valid for 365 days, however, any athlete participating in future seasons must complete a "Health History Questionnaire".   

    NOTE: 2019-20 athletic forms will be available via Parent Portal on June 3, 2019. 

    Attention Sparta Middle School's Incoming Class of 2026 and Returning Athletes:

    All SMS athletes participating in Cross Country during the Fall of 2019, MUST have their physical forms into the Nurse's Office NO SOONER than June 15, 2019 and NO LATER than August 22, 2019. If you are submitting your physical paperwork after June 21, 2019 and during the Summer months, please drop this paperwork in the Main Office of Sparta Middle School. *NOTE: Forms submitted after that date may cause a delay in the athlete's participation in practices.
