All students are required to have an annual physical examination by their private physician for participation in the Sparta High School Athletic Program. The physical examination must be completed on the school medical form*. The form consists of four parts; Parts A, B, C and D. The examining physician must complete all of Part B, including vision and hearing.
All medical forms for sports must be submitted to the high school no later than 2 weeks before the start of the season.
Students will not be allowed to participate in a sport (practice or games) unless the physical examination** requirement has been met.
Incomplete forms are not acceptable and will delay sport eligibility.
As required by N.J.A.C. 6A:16-2.2, the school physician will review the physical examination, and indicate with his/her signature that your private physician has medically cleared your son/daughter for sports participation.
*Additional school medical forms are available in the Health Office, Main Office, and Athletic Office at Sparta High School.
**The school recommends retaining a completed copy of the physical exam forms for your records.

IMPACT BASELINE TESTING (CONCUSSION TESTING) - 2010
We strongly suggest that you have your child take this baseline concussion test. This test will help the Athletic Trainer evaluate your child in the future if ever the need arises. The baseline test is good for 2 years so if they took the test last year, they do not have to take it again until next.
Students will need to bring a permission slip signed by parent in order to participate in concussion testing. Permission slip can be downloaded from below.
