Download and fill out the following forms if you have not done so. Please follow instructions on each form.

Genesis Parent Module Registration Form

to download a registration form. Registration will allow you to access your child's grades online.

Computer Network Acceptable Use Policy Form

for school required form. Please read each section and have your child initial/sign where indicated along with your signature, and have him/her return it to his/her homeroom teacher no later than September 30, 2011. If this form is not returned, the student will

be denied access to use all school computers.

Honeywell Instant Alert Form

for form. Please take time to complete or update your electronic

communication profile sheet. All families should register to get emergency and important information.

Publicity Consent Form

for the Publicity Consent Form. An important part of Sparta High School is highlighting the achievements of students. If you would like to change your original choice, we encourage you to download and fill out the Publicity Consent Form and have your child bring it to the Attendance Office. New parents to our district must fill out and return this form.

PSTO Membership Form
