Horizon Blue Cross/Blue Shield of NJ – Direct Access Plan
$5 co-pay for “in-network” doctors (includes specialists—no referrals needed)
Well care (physicals) covered.
Out-of-network benefits are also included.
$100 deductible per person – Reimbursement 70%.
(When using out-of-network doctors, you must file the claims.)
Horizon Blue Cross/Blue Shield of NJ – Traditional Plan
$150 deductible per person – Reimbursement 80%
Prescription coverage is included on your Horizon ID card.
A “mail-in” prescription plan is also available for maintenance drugs (allergy, asthma, high blood pressure, etc.). A 90-day prescription with three refills is required. (Forms are available in my office - allow 2 weeks for new prescriptions.)
“SmartEyes” is a discount program offered to Horizon members. It provides discounts on eye exams, glasses and contacts when using a participating office (Pearl Vision Center, Sears, JC Penny, Target). Locally, Total Eyecare on Route 15 participates. Call 800-424-1155 for the nearest location.

Any dentist can be used with our plan. Two cleanings per year are covered at 100%.
